Pesticide Notification Request Form

I understand that, upon request, the Mill Valley School District is required to supply information about individual pesticide applications at least 72 hours before application. I request that the District notify me before any pesticide application at schools. I further understand that, as a courtesy, the District will notify me of any pesticide applications that the City of Mill Valley may apply to fields adjacent to the schools.
