To: Parents Registering Children for Kindergarten

RE: IMMUNIZATION AND HEALTH EXAMINATION  REQUIREMENTS

County health and school officials require that parents obtain a medical and immunization checkup for their child before entering kindergarten, either from your private physician or Marin County Health Department.

IMMUNIZATION AND HEALTH REQUIREMENTS

5 DPT The last dose given after two years of age 

4 Polio The last dose given after two years of age (The month and year of each vaccine must be listed and verified)

3 Hepatitis B To be completed at kindergarten entry

2 MMR  (Measles, Mumps and Rubella) recommended age is 15 months, but any time after first  birthday meets legal requirements. The month, day and year must be listed and verified.

Chicken pox vaccine or proof of disease 

Physical Health Exam Not earlier than 6 months before entering Kindergarten 
(proof of a tuberculosis clearance is required)

Children will be excluded from school if the health requirements are not verified on or before the first day of school. (California State Health and Safety code 3380-3390)

School Law does allow for personal or religious exemptions, but a waiver must be signed and kept in your child’s record file.

Low income families may be able to obtain the required examination free of charge if their child is not on Medi-Cal. To find out whether you are eligible, call the Child Health and Disability Prevention office at 499-6886.
