1. An original or certified (county seal) birth certificate

The proofs must be no older than 60 days

UNPAID so that
billing and site address are listed on the document. Please note: If you receive

A TB test.
Required ONLY if your family is moving to
Mill
Valley
from countries in
Asia,
the Middle East, Africa, Latin America,
Eastern Europe, or the Pacific
Islands. The test must have
been administered upon arrival in the
United States. Written proof of a negative test result will be required before the child can
start school.

on registration days at the District Office.
Please
note: Your child cannot be registered
until we have all of these documents with the exception
of the health and dental forms.
The health form and dental forms need to be completed prior to school
starting but you will not need to furnish them on the day of
registration.

How
is school placement decided?
A
common misperception is that parents “choose” their child’s school.
Parents have the opportunity to state a preference for a particular school.
Actual placement, while taking preferences into consideration is dependent on
the following criteria:
- Siblings
of currently enrolled students residing in the District are considered residents
of the sibling’s school and will be enrolled in that school if they wish and
if there is space. The currently enrolled sibling must be in attendance at the
school when the new enrollee enters the school.
- Facility
capacity and class size
- Proximity
of residence to the school of choice
- Identified
special education needs
- Balance
of class size across the District
When
will I be notified?
Students registered on the
registration dates in January and February 2010 will be notified of school
assignment by May 1, 2010. Students
registered between February 5, 2010 and June 17, 2010 will be notified of school
assignment by July 1, 2010. Students registering after June 17, 2010 will be
notified of school assignment prior to the opening of school.
What
if my child is not placed in the preferred school?
When
demand exceeds capacity a waiting list is established and parents may exercise
their option to have their child’s name placed on the list. Additional
placements in a school may be taken from the waiting list but may not be made
until August 2010.
