Student Registration & Enrollment Forms

Please complete the forms applicable to your child's grade level and bring them, with the required documents as outlined in Guide to Registering to the Mill Valley School District Office.

Kindergarten

 5 Forms to be Completed:

First 3 Forms must be completed by day of registration. Health Form and Dental Form need to be in before school starts. See details below. 

1. Kindergarten Registration Form

2. Home Language Survey

3. STAR Parent & Student Demographic Form

4. Kindergarten Health Form (2008/2009 school year registrants, only.) To be completed by child's doctor.  Physical exam must be done after March 1, 2008.  

5. Kindergarten Dental Health Letter & Form - English  Dental exam must be done anytime after September 21, 2007  or Kindergarten Dental Health Letter & Form - Spanish

Kindergarten Immunization Letter  (not a form- 2008/2009 school year registrants, only)
