Student Registration & Enrollment Forms

Please complete the forms applicable to your child's grade level and bring them, with the required documents as outlined in Guide to Registering to the Mill Valley School District Office. Forms are also available for pick up in the District Office at 411 Sycamore Ave. Hours (Mon-Fri, 8-4 PM. Office closed from 12-1 for lunch.)

KINDERGARTEN FORMS

There are 6 forms to be completed.  The first 4 forms must be completed by day of registration. Health Form and Dental Form need to be in before school starts.  See details below.  

1. Kindergarten Registration Form

3. Declaration of Residency
4. STAR Demographic Form  
5. Kindergarten Health Form To be completed by child's doctor.  Physical exam for 2013/14 registrants must be done after March 1, 2013. 

6. Dental Health Form Kindergarten Dental Health Letter & Form - English.  Dental exam must be done anytime after September 1, 2012

7. *If English is not students' first language then you will also need to complete the Primary Language Survey

   Do ALL 7th grade students need to meet the pertussis immunization requirement?
   Yes. All students going into 7th grade must have proof of having had the Tdap booster shot unless they have a documented exemption. This includes current students, new students and transfer students. Many students have already received the vaccine and simply need to supply proof to the school, so check with your doctor or provider.
