Student Registration & Enrollment Forms

  • Parents, please read the School Placement FAQs before beginning the registration process.

    The Mill Valley School District implemented an online registration process that is intended to be more user and environmentally-friendly. For those without Internet access, please contact the District Registrar, Leni Moore at 415-389-7712 for alternate arrangements.

    Important: You can begin the online registration process as soon as you are physically living full-time in the Mill Valley School District.  Simply owning property or holding a lease does not qualify as residency.
      
    There are 4 steps in the registration process.  PLEASE MAKE SURE YOU COMPLETE THEM ALL!
     

    Any information you submit for the Online Registration Form is secure. YOU MUST COMPLETE THE ONLINE REGISTRATION FORM AT LEAST 48 HOURS BEFORE YOUR REGISTRATION APPOINTMENT.

    Download and complete the additional forms applicable to your child's grade level (listed below). Print and bring these forms to your Registration Appointment (see #4).

    Gather other required documentation, including proof of age, proof of residency and proof of immunizations, as outlined in Guide to Registering for the Mill Valley School District. Bring these documents to your Registration Appointment.

    If you are registering for the 2020-2021 or 2019-20 school year, please make a Registration Appointment by contacting Leni Moore at lgmoore@mvschools.org or 415-389-7712.  A parent/guardian must show up to the Registration Appointment to submit and sign appropriate forms.


    EARLY INTERVENTION FORMS

    There are up to 3 forms to be completed.  All forms must be completed/submitted by your Registration Appointment. 

    1. Release of Information Form
    2. Declaration of Residency Lessor Supplemental Form (for renters only)
    3. Proof of Immunizations - Please provide your current Immunization Record from your child's doctor, listing all immunizations to date.

     


    KINDERGARTEN FORMS

    There are up to 5 forms to be completed.  The first 3 forms must be completed by your Registration Appointment.

    1. Release of Information Form
    2. Declaration of Residency Lessor Supplemental Form (for renters only)
    3. Proof of Immunizations (See Kindergarten Immunization Letter below.)
    4. Health Form To be completed by child's doctor.  Physical exam for 2020-21 registrants must be done after March 1, 2020 and is due no later than December 12, 2021 (1st Grade). 
    5. Dental Health Form Kindergarten Dental Health Letter & Form - English.  Dental exam may be done at any time and is due no later than December 12, 2021 (1st Grade).

    Do ALL 7th grade students need to meet the pertussis immunization requirement?

    Yes. All students going into 7th grade must have proof of having had the Tdap booster shot unless they have a documented exemption. This includes current students, new students and transfer students. Many students have already received the vaccine and simply need to supply proof to the school, so check with your doctor or provider.
